Osteotomoclasis in the correction of Hong bone deformities

Abstract
Since Macewen; in 1878 described what is now known as the cuneifrom osteotomy, numerous methods have been devised for the.correction of deformities of the long bones.
The problem of preventing displacement of the osteotomy fragments has been responsible, more than other single factor, for the invention of many types of osteotomies.
In recent years,internal fixation by means of metal plates and screws, etc, has been popular in countering this problem.
However, the necessity of large incision and dissection, increased danger of infection, the usnal hazards associated with more difficult surgery, the difficulty in obtaining and maintaining an exact amount of correction desired, and the possibility of delayed and nonunion plus the necessity of removing the metal at a later date, particularity in the children, have all detracted considerably from the mechanical advantages obtainable by internal fixation.
The osteotomoclasis method consists simply in cutting the long bone, almost but not quite entirely in two, near or at the site of maximum deformity, and then completing the osteotorry by manual esteoclasis. at a later period. No internal fixation is necessery and immobilization is achieved by cast only. At times, half pin or half pins incorporated in the plaster may be added in order to control the fragments at time of correction.by osteoclasis.
Presented here are 8 cases of long bone deformities and 2 cases of Legg-Perthes disease treated by osteotomoclasis tecluiigne. at the Seoul National University Hospital during the last 3 years, from 1963 through 1966.
We believe that the rationale of this method is. sound and the technique simple, easy and accurate to execute and that the method has a wide and versatile place in the correction of long bone deformities.
